Prioritizing Minority Health in SouthEast San Diego

In the heart of San Diego County, JIREH Providers is working diligently to address the significant health disparities faced by communities of color. We are devoted to equalizing access to quality healthcare and pioneering preventative care initiatives, especially for chronic conditions that disproportionately affect these communities.

The Challenge

Communities of color are disproportionately affected by chronic diseases, with BIPOCs (Black, Indigenous & People of Color) being nearly 50% more likely to have a stroke and Latinos being 50% more likely to die from diabetes than white Americans. Hypertension, obesity, asthma, and cancer also pose considerable threats to these communities. These health disparities are further amplified by social determinants of health, such as poverty, housing, lack of access to quality healthcare, and a shortage of culturally appropriate health providers and information.

Our Response

As a mobile community clinic, we understand the significance of taking preventative healthcare to the people, where it’s most needed. We partner with trusted providers  in the community who have a reputable track record for bringing results. Our Health Management and Preventative Care Program provides targeted education and screenings for chronic diseases, including hypertension, asthma, obesity, nutrition-related issues, diabetes. We’re also advocating for equitable allocation of health resources and fair access to care. We also empower the community through quarterly access to CPR/AED training.

Our Impact

Our proactive approach to healthcare is transforming lives. For every person who learns how to manage their blood pressure, every individual who discovers they are pre-diabetic and takes steps to reverse it, we are chipping away at the health disparities entrenched in our system. As members of the Central Region Leadership Team and San Diego County Equity Task Force, we’re also helping to shape policies and practices that promote health equity.
